Southwest Medical Center will open a new primary care clinic, Liberal Family Medicine Care Center, to ensure continued access to quality health care for the community as the current Southwest Family Medicine clinic location closes.
Family nurse practitioners Heidi Riley, APRN, FNP-C, and Tiffany Landa, APRN, FNP-C, will continue providing care for their patients and will become employed providers of Southwest Medical Center. Both providers will continue seeing patients at the current Southwest Family Medicine location through Thursday, February 26.
Following the clinic’s closure, there will be approximately a one-month transition period as preparations are finalized for the new Liberal Family Medicine Care Center location at 121 E. 11th Street, directly across the street from the previous practice.
Patients who need prescription refills, medical records, or an appointment prior to the transition are strongly encouraged to contact the clinic before the closing date of Thursday, Feb. 26.
During the one-month transition period, the clinic will not be seeing patients while preparations are completed for the new location.

Liberal Family Medicine Care Center will welcome both existing and new patients and accepts most major insurance plans, including Blue Cross Blue Shield, Medicare, and Kansas and Oklahoma Medicaid. Clinic hours will be Monday through Thursday from 8 a.m. to 5 p.m. and Friday from 8 a.m. to 12 p.m.
Tiffany Landa will begin seeing patients at the new location March 30, and Heidi Riley will begin seeing patients there April 20.
Riley brings more than 22 years of experience as a family nurse practitioner serving the Liberal community. She earned her Master of Science in Nursing from the University of Kansas and has provided care locally for many years, including service at Southwest Family Medicine and the Seward County Jail.
Landa has served the Liberal area as a family nurse practitioner for 14 years. She earned her nursing degrees from Colorado State University and has worked in family medicine, urgent care, and public health settings in Southwest Kansas. Her professional focus is caring for patients as whole individuals while providing compassionate and family-centered care across the lifespan.
